Instructions to complete student survey

Thank you, for taking the time to complete our survey. We appreciate your participation. The survey should only take a few minutes.

  • You do not have to answer every question.
  • Complete only those questions that you are comfortable answering.
  • Some questions allow you to select multiple answers.
  • Others will only allow one answer per question.
  • Answer the text boxes as completely as you need.
